河鱼腹疾 is a Chinese idiom (chengyu) meaning “to suffer from diarrhea”. To have loose bowels.
In the winter of 597 BC, the state of Chu attacked and besieged the city of Xiao. Shen Shuzhan, a high-ranking official of Chu, and Huan Wushe, a high-ranking official of Xiao, were good friends. Huan Wushe was still inside the besieged city.
Shen Shuzhan could not openly rescue him, so he used the cryptic phrase 'he yu fu ji' (river fish's abdominal illness) to secretly signal his friend to hide in a dry well to escape danger. Eventually, he successfully rescued his friend.
